NEW CLIENTS:
To facilitate the beginning of the ‘discovery’ process for Bio-Integrative Mapping framework, two forms are required for completion: Client Intake Form and the Initial Questionnaire and Background Details Form. Please forward completed forms to the confidential inbox of transformuniquely@wavedirect.net at least 3 days prior to your first scheduled session.
Virtual sessions will be held via Zoom, Skype, or Facebook video.

CANCELLATION POLICY:
Appointments cancelled within 2 hours of the scheduled session time or missed completely are subject to a minimum $50 fee due immediately and must be paid prior to rebooking.
Excessive abuse of appointment times will result in a 100% fee charge of the scheduled appointment time.
Cancellations may be made by phone or email.
